The old rules of the sea and seamanship still apply. Keep a really good watch and intentionally eliminate opportunities for error.

A very sad story. Plenty of senior navy officers to hold accountable. A lot of heroic action by the crew which does not surprise me. The troops rarely fail to deliver.... I sure hope the Navy is getting itself sorted out but I fear this was is years in the making.
